Monthly Periodic Fees:
Individual
This fee is for ongoing primary care services, per member.
- 18-39 years of age – $75 per month.
- 40-64 years of age – $125 per month.
- 65-Older – $150 per month.
Members receive discounts on other services and items offered in the clinic.
Appointments
One-time appointments are $200 and includes 30 days of care.
Weight loss consultation with 6 months of care is $300.
Weight loss consultation with 3 months of care including personalized plan and visits with nutritionist is $600.
Hormone replacement therapy consultation with 6 months of care is $300.
Hormone replacement therapy consultation for BioTe hormones is $250. Procedure fee is $400.
